Application Process
Working Visitor Application Process

In order to help you keep track of what is needed within the Working Visitor application process, we have included a step by step format to follow.

Step 1 - Application Forms
(left click to view forms if your browser supports this feature, or right click to save the file to your hard drive for later viewing and printing)

Fill out, read, sign, and return the following to HCJB:
1. Working Visitor Application (Online, MS Word, PDF)
2. Statement of Faith (MS Word, PDF)
3. Liability And Code of Conduct (MS Word, PDF)
4. Malpractice form (Doctors only) (MS Word, PDF)

These forms can be obtained on this website by following the links. Fill out and return them via email to , apply directly online, or if you would rather, you can also print out these forms and send them by:

1. Mail:

Step 2 - Reference Forms
Send out three reference forms (MS Word or PDF):

The PDF form is meant to be printed and sent by regular mail, and the completed form sent to the Mail: address found a few lines above.

The MS Word file is meant to be opened, edited, a saved. Then that person can send the modified Word document back to as an attachment.

Send copies to:
1. one reference to your pastor
2. Two references to friends or colleagues.

Step 3 - Diploma and Certificate (Doctors only)
Send a notarized copy of your medical school diploma and specialty certificate.
You can either send them by mail, scanned and emailed or fax. (If sent by fax or email, please bring the notarized copy with you when you come.
There is no need to bring one with you if you send it by mail though.)


Once Accepted
This is just the beginning! once you have been officially accepted by HCJB Global to come as a Working Visitor, you will be able to continue on in your planning for your trip. The “Destination Ecuador” guide (MS Word or PDF)will give you all the needed details in order to prepare for your trip, including detailed information about the following topics that you should keep in mind.

Passport - If you don’t already have a passport you will need to start the process as soon as possible. It could take quite a while (6 - 8 weeks) in order to obtain one.

Visa - If staying for longer than 6 months you will need to start the paperwork for a visa before you leave your country. “Destination Ecuador” will tell you all you need to know.

Immunizations – Depending on your destinations, some immunizations may be recommended. With some of the immunizations needing to take place over a 6-month period, it is recommended that you start early. (See “Destination Ecuador” for recommendations)
